Comprehensive Reentry Case Management Strategies In offenders
This course was developed as a way to share our unique experience in reentry case management. The national average recidivism rate within one year of being released is 65%. Our recidivism rate for the 601 high risk/high need participants we have worked with is 19.9%. This Comprehensive Reentry Case Management program begins with the administration of several standardized screenings and assessments prior to entering the prison reception center to determine level of risk and identify primary needs. Pre and post-release case management is provided by the same team of Case Managers who meet with the participants during their incarceration and pick them up at the prison gate upon their release. Once in the community, participants meet with their community case manager on a regular basis to receive services.
After completing this course, participants will be able to:
- Apply a series of validated and reliable assessment tools to determine the nature and severity of disfunction in multiple life domains (risk and needs assessment);
- Develop an initial Life Plan - a joint case management document that will be reviewed and adjusted throughout, starting at pre-sentencing all the way to post-release;
- Evaluate progress and make necessary changes based on the intermediate assessment finding;
- Develop effective working partnerships with other community organizations and local authorities.
